Registration Checklist:


For Each Child:

  • Birth certificate

For Parent/Guardian:

  • Drivers license
  • If no driver's license: Utility bill or lease agreement

Proof of income:

  • 2 weeks worth of paystubs
  • Documentation of any unearned income:
  • Unemployment
  • Social Security
  • Child Support

If applicable:

  • Custody plan or letter (for co-parenting situations)
